Evolve Therapy Services is seeking a Director of Rehabilitation for an in-house Skilled Nursing Facility in Freeport, IL. The role leads the therapy program, ensuring high-quality services and regulatory compliance under the guidance of the Regional Director of Operations.
Candidates must hold a degree in PT/OT/SLP and possess an active license in state(s) of practice, with PDPM/MDS experience preferred. Strong leadership and communication are essential.
