Job description
Evolve Therapy Services in Laona, Wisconsin seeks a Director of Rehabilitation (PT/OT/SLP) to oversee in-house therapy services at an affiliated Skilled Nursing Facility. You will lead the therapy program, ensure regulatory compliance, and drive quality and productivity in collaboration with the facility team.
Provide leadership to PT/OT/SLP staff, manage PDPM/MDS processes, and maintain licensure. This role requires strong communication, strategic thinking, and a patient-centered approach.
