Job description
The White Plains Center for Nursing in White Plains, NY, seeks a Director of Social Work to lead the social services team, oversee case management, discharge planning, and care planning for 88-bed facility residents.
You will collaborate with the Administrator and care teams to tailor plans per resident, ensure MOLST and Sigma Care compliance, and drive quality initiatives to improve outcomes and satisfaction.
