General Job Summary : The Director, Value-Based Care is responsible for leading the strategy, direction, and organizational alignment of value-based care initiatives across the organization. This role establishes priorities, builds key clinical and operational relationships, and ensures alignment of care delivery models with organizational goals related to quality, patient outcomes, and cost efficiency.
The Director partners closely with executive, finance, clinical operational leadership and external partners to advance value-based care strategies, aligning efforts across primary care, specialty care, and ancillary services to support sustainable, high-performing care delivery. Director is responsible for building alignment and accountability across cross-functional teams to drive outcomes.
Primary Job Responsibilities
Value-Based Care Strategy & Leadership Leads value-based care strategic planning and governance across the organization
Translates payer, ACO, and population health objectives into enterprise priorities, programs, and goals
Aligns value-based care strategies with organizational goals to maximize team performance
Identifies opportunities to expand and strengthen value-based care models
Clinical & Executive Partnership Builds and maintains strong relationships with physicians, clinical leadership, and executive stakeholders
Serves as a subject matter expert and a strategic advisor to executive leadership on value-based care initiatives and performance
Supports evaluation of value-based contract opportunities with finance and managed care leadership
Organizational Alignment & Integration Ensures alignment of workflows, resources, and care delivery models across primary care, specialty care, and ancillary services
Partners across clinical, operational, financial, analytics, and technology teams to remove barriers and drive execution in support of collaboration and outcome performance
Leads, develops, and mentors VBC team members, establishing clear priorities, performance expectations, and accountability
Drives consistency and coordination across operational functions
Identifies and addresses system-level barriers to performance
Performance & Outcomes Establishes key performance metrics and monitors organizational performance to identify opportunities and communicate results to executive and clinical leadership
Partners with Finance and Actuarial leadership to monitor and forecast VBC performance, identify financial risks and opportunities, and evaluate program ROI
Translates clinical, utilization, and financial performance data into actionable strategies for clinicians, practices, and operational teams
Drives continuous improvement through data-informed decision-making
Ensures sustainability and scalability of value-based care programs
Education and Experience
Required Bachelor's degree in Healthcare Administration, Business Administration, Public Health, or a related field
Minimum of 8+ years of progressive leadership experience in healthcare operations, population health, value-based care, or a related environment
Experience with value-based care models (e.g., ACOs, risk-based contracts, population health programs)
Proven experience leading cross-functional initiatives and managing complex, organization-wide programs
Demonstrated ability to partner effectively with physicians, executives, and operational leaders
Preferred Master's degree (MHA, MBA, MPH, or related field)
Strong financial and analytical experience with forecasting, ROI analysis, and performance measurement using claims data
Experience in a physician-led or multi-site ambulatory environment
Physical Requirements Work consistently requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
Must be able to lift and support weight of 35 pounds.
Ability to concentrate on details.
Use of computers for long periods of time.
