Job description
ACLD is seeking a Medical Coordination Specialist in Huntington, NY to coordinate medical care for individuals with disabilities at a residential site. You will support health and safety, mentor residents, and assist with daily living tasks while promoting independence.
Responsibilities include medication administration per AMAP, weekly nursing coordination, scheduling appointments, and maintaining documentation such as MARs and treatment notes.
