Job description
AbsoluteCARE Medical Center & Pharmacy is seeking a Transitional Care Manager to join the integrated community care team. You will coordinate discharge planning and post-discharge care for members admitted to inpatient facilities, working with health plans, PCPs, and community resources to meet individual needs.
The role emphasizes collaboration, documentation, and adherence to accreditation standards, with travel across local facilities to support patient transitions and reduce readmissions.
