Job description
Hartford Hospital is a leading teaching hospital and Level 1 Trauma Center, committed to safe, patient-centered transitions of care. As the Discharge Lounge RN, you will coordinate transfers of discharge-ready patients from inpatient units to the Transition Lounge, working with Case Coordination, EMS, and support services to maintain throughput and capacity.
You will lead the Transition Lounge team, ensure timely handoffs, monitor barriers to discharge flow, and uphold Hartford HealthCare values
