Job description
La Palma Intercommunity Hospital is seeking a case management professional to coordinate department activities, assist in discharge planning, and support case management referrals and authorizations. The role involves Medicaid documentation and data gathering to develop safe discharge plans.
The ideal candidate will have knowledge of discharge planning and utilization management, basic computer skills, and a high school diploma, with hospital experience preferred.
