Job description
Mediko Inc is seeking a Discharge Planner to manage discharge processes for patients in a correctional healthcare setting. You will identify needs, coordinate pre-release planning with healthcare teams, and ensure timely follow-up with community resources and providers.
You will document all activities in the health record, maintain compliance, and liaise with the Sheriff’s Office and community agencies to support smooth transitions and appropriate post-release care.
