Job description
College Medical Center Phoenix is seeking a full-time Discharge Planner to join the Case Management team in Phoenix. You will facilitate care along the continuum, coordinating resources and ensuring appropriate use of services while respecting patient self-determination.
The role requires a BS in Sociology or Psychology, at least one year in behavioral health, and BLS certification on hire. CPI certification must be obtained within 30 days of hire.
