Job description
Discharge Planning Clerk
If you are caring, someone who likes to do a good job and wants to work for an employer who values these same traits, then CVRMC wants you for the position as Discharge Planning Clerk. Under general supervision coordinates the integration of Discharge Planning & Swing Bed level care. This position functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.
Qualifications:
Social worker or RN preferred, willing to trainAt least one (1) year experience working with medical patients preferredMust possess assessment skills, exhibit sound judgment and be able to communicate effectivelyMust be knowledgeable of community resources and be able to network effectively within the communityMust be knowledgeable regarding Medicare and AHCCCS regulations/requirements
