Job description
River Edge Behavioral Health is seeking a Full Time Discharge Planner Group Leader to work under the supervision of the Adult Clinical Nurse Manager. The Discharge Planner will lead skill training groups to enhance recovery/crisis management skills of behavioral health clients at the Crisis Stabilization Unit.
The Discharge Planner will act as client advocate to coordinate discharge plans and obtain needed resources for continued recovery after discharge from the Crisis Stabilization Unit.
