Job description
FirstHealth of the Carolinas seeks a Discharge Planner (RN, MSW, or BSW) to coordinate comprehensive discharge plans with patients, families and the care team. You will assess discharge needs, coordinate referrals, and promote timely, safe discharges across the hospital system.
