Job description
FirstHealth of the Carolinas is seeking an experienced Discharge Planner (RN, MSW, or BSW) to coordinate comprehensive discharge plans in Raeford, NC. This is a non-remote, on-site role within a nationally recognized health system.
The position requires nursing or social work credentials with 3+ years of related experience (RN with 3+ years or ADN with 4+ years). Medical Terminology is required; Case Management certification is preferred. Sign-on bonus and relocation assistance offered.
