Job description
St. Mary's Medical Center in Huntington, WV is seeking a full-time Discharge Planning Specialist. The DPS will assist the Care Facilitator and Social Worker with discharge planning to promote timely and safe discharge services, coordinating care across hospital teams.
Under supervision, the DPS coordinates home health, DME, rehab placement, and community resources, ensuring smooth transitions for patients and families, with a focus on clear communication and efficient service delivery.
