Job description
St. Mary's Medical Center in Huntington, VA is seeking a full-time Discharge Planning Specialist to support discharge planning activities and coordinate patient transitions.
Under supervision, you will work with the Care Facilitator and Social Worker to arrange home health, DME, rehab placement, and connect patients with community resources. The role requires a High School Diploma, BLS, and strong communication skills; hospital experience preferred.
