Job description
Saint Clare's Health in Denville, NJ is seeking a Case Management Coordinator to support discharge planning under the guidance of the Case Manager/Social Worker. You will assist with developing and implementing discharge plans and help with case management referrals and authorizations.
The role also involves coordinating Medicaid submissions for on-site review and interviewing patients to develop safe, patient-focused discharge plans. A current BCLS certification is required at hire.
