Job description
Louisiana Behavioral Health, a partnership between Ochsner LSU Health Shreveport and Oceans Healthcare, seeks a Discharge Planner to coordinate discharge planning for inpatient programs with physicians, therapists and nursing staff.
The Discharge Planner works with the Performance Improvement Coordinator to ensure timely reviews, accurate documentation, and family communication while adhering to Joint Commission and state regulations.
