Job description
Rimrock Foundation seeks a Discharge Planner to provide therapeutic supervision and coordinate services for adults in inpatient treatment. You will ensure safety and security of the facility, adhere to HIPAA and 42 CFR Part 2, and support counselors as needed.
The role requires organizing discharge plans, arranging continuing care, and collaborating with the therapeutic team to arrange housing, legal, and financial supports. Strong communication and confidentiality are essential.
