Responsibilities:
Provide direct patient care, including administering medications, performing wound care, and conducting vital signs
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Monitor patient progress, reporting any changes or concerns to the RN or other healthcare professionals
Educate patients and their families on disease management, self-care techniques, and medication administration
Maintain accurate and detailed records of patient care and progress
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
Current LVN certification from the State of Texas
Minimum 0 years of experience in a healthcare setting
Ability to work flexible schedules, including weekends
Strong communication and interpersonal skills
Ability to prioritize tasks effectively and manage multiple responsibilities
Physical stamina to lift, move, and transfer patients as needed
