Description
Conduct thorough evaluations of patients in both inpatient and outpatient settings and create tailored treatment plans
Implement physical therapy techniques that enhance mobility, strength, balance, and overall function
Collaborate closely with nurses, physicians, case managers, and other rehabilitation experts to optimize patient outcomes
Provide education to patients and their families regarding treatment goals, care transitions, and home exercise routines
Maintain accurate documentation of evaluations, treatments, and patient progress, adhering to facility and regulatory guidelines
Engage actively in care team meetings and interdisciplinary conferences to ensure cohesive and coordinated care
Education
Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
Current state licensure as a Physical Therapist
CPR certification preferred
Additional specialized certifications (such as neurological or geriatric) are advantageous but not mandatory
Skills
Relevant clinical experience is highly favored
