Perform assessments for patients in inpatient or outpatient settings and formulate personalized treatment strategies
Deliver physical therapy services concentrating on mobility, strength, stability, and daily functionality
Work in tandem with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
Educate patients and their families about treatment objectives, discharge plans, and at-home exercises
Maintain meticulous records of evaluations, treatments, and patient progress in alignment with organizational and regulatory standards
Engage in care team meetings and interdisciplinary discussions to ensure holistic patient care
Doctorate in Physical Therapy (DPT) or an equivalent accredited degree
State license required for Physical Therapist
Preferred: CPR certification
Additional specialized certifications (neurological, geriatric) are advantageous but not mandatory
Prior experience is strongly preferred
Able to address patients with intricate medical and mobility challenges
Excellent communication and teamwork abilities
Proficient with digital patient management and documentation systems
