Description
Assess patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to individual needs.
Deliver targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
Work collaboratively with nurses, physicians, case managers, and other rehab professionals to ensure optimal patient outcomes.
Guide and educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercise routines.
Document evaluations, treatment progress, and care strategies in compliance with facility and regulatory standards.
Engage in interdisciplinary care team rounds and conferences to ensure cohesive patient care.
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
Certifications
Valid state licensure as a Physical Therapist.
CPR certification is preferred.
Specialized certifications (neurological, geriatric) are advantageous but not mandatory.
Skills
Prior experience strongly preferred.
Ability to address complex medical and mobility challenges.
