Job Description
Conduct thorough evaluations of patients in either inpatient or outpatient environments and craft tailored treatment plans
Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Work closely with nurses, physicians, case managers, and a diverse range of rehab professionals to elevate patient outcomes
Engage and educate patients and families on treatment objectives, discharge planning, and at-home exercises
Maintain comprehensive documentation of evaluations, treatments, and progress that aligns with facility and regulatory standards
Engage in interdisciplinary care team rounds and conferences to ensure seamless care
Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
Must hold a state license as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience is strongly preferred
Ability to effectively treat patients with complex medical and mobility challenges
Excellent communication and collaborative skills
Comfortable navigating digital charting and clinical documentation systems
