Conduct thorough evaluations of patients in both inpatient and outpatient environments and formulate tailored treatment strategies. Implement physical therapy interventions aimed at enhancing mobility, building strength, improving balance, and restoring functional abilities. Collaborate seamlessly with nurses, physicians, case managers, and a variety of rehabilitation professionals to achieve optimal patient outcomes. Provide education to patients and their families on therapeutic objectives, discharge planning, and at-home exercise routines. Accurately document evaluations, treatments, and patient progress in alignment with facility protocols and regulatory standards. Engage in care team discussions and interdisciplinary meetings to ensure smooth, coordinated patient care.

Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution. State licensure as a Doctor of Physical Therapy. CPR certification is preferred. Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not mandatory.

Prior experience is highly preferred. Ability to manage patients with intricate medical and mobility challenges. Excellent communication and teamwork capabilities. Aptitude for utilizing digital charting and clinical documentation tools.
