Description
Assess patients in either an inpatient or outpatient context and craft tailored treatment plans that meet their specific needs
Administer targeted physical therapy techniques that prioritize strength, balance, mobility, and overall functionality
Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation specialists to maximize patient outcomes
Provide patient education for families on therapeutic milestones, discharge criteria, and at-home exercise routines
Accurately document evaluations, treatments, and patient progress adhering to both facility and regulatory standards
Participate actively in interdisciplinary team discussions and care rounds to ensure seamless patient care
Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
Certifications
Valid state licensure as a Physical Therapist
CPR certification is preferred
Any specialized certifications, such as in geriatrics or neurology, would be advantageous but are not required
Skills
Previous relevant experience is highly valued
Competence in treating patients with complex medical conditions and mobility challenges
Excellent communication skills and teamwork mindset
Familiarity with digital medical record systems and clinical documentation processes
