Description
Conduct assessments of patients in both inpatient and outpatient environments, designing personalized treatment plans tailored to their needs
Implement physical therapy interventions emphasizing mobility, strength, balance, and functional enhancements
Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize therapeutic outcomes
Educate patients and their families regarding therapeutic objectives, discharge plans, and home exercise regimens
Maintain comprehensive documentation of evaluations, treatments, and patient progress per facility and regulatory standards
Engage in care team discussions and interdisciplinary meetings to ensure seamless, integrated care
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
Current state licensure as a Physical Therapist
Preferred CPR certification
Specialty certifications (i.e., neurological, geriatric) are advantageous, though not mandatory
Skills
Previous experience is strongly preferred
Capability to manage patients with intricate medical and mobility challenges
Exceptional communication and teamwork abilities
Comfort with digital charting and clinical documentation platforms
