Description
Assess and evaluate patients within both inpatient and outpatient settings, crafting customized treatment plans.
Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function.
Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes.
Guide patients and their families through therapeutic goals, discharge processes, and recommended home exercises.
Maintain clear documentation of evaluations, treatments, and patient progress according to facility and regulatory standards.
Engage in care team rounds and interdisciplinary discussions to ensure cohesive care strategies.
Education
Doctorate in Physical Therapy (DPT) or an equivalent accredited qualification.
Certifications
Active state licensure as a Physical Therapist.
CPR certification preferred.
Additional specialized certifications (neurological, geriatric) are a bonus but not mandatory.
Skills
Relevant prior experience is highly desirable.
Capable of treating patients with multifaceted medical and mobility challenges.
Exceptional communication and teamwork abilities.
Familiarity with digital charting and electronic health record systems.
