Description
Conduct evaluations in both inpatient and outpatient environments to create personalized treatment plans for patients
Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functioning
Work collaboratively with nurses, doctors, case managers, and other rehabilitation professionals to ensure optimal recovery outcomes
Guide patients and their families through therapeutic objectives, discharge strategies, and at-home exercise routines
Maintain thorough documentation of evaluations, treatment processes, and patient progress in line with facility regulations
Engage in care team discussions and interdisciplinary meetings to foster cohesive care delivery
Education
Doctorate in Physical Therapy (DPT) or equivalent qualifying degree from an accredited institution
Certifications
Current state licensure as a Physical Therapist
CPR certification is preferred
Specialty certifications (neurological, geriatric, etc.) are beneficial but not mandatory
Skills
Prior experience in a clinical setting is highly favored
Aptitude for treating patients with intricate medical scenarios and mobility challenges
Excellent communication and collaborative skills
Competence in using electronic health records and clinical documentation tools
