Description
Conduct thorough evaluations of patients in both inpatient and outpatient environments and create customized treatment plans
Deliver targeted physical therapy interventions focused on enhancing mobility, strength, balance, and functionality
Work collaboratively with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes
Provide education to patients and their families regarding therapeutic goals, discharge processes, and at-home exercise regimens
Maintain detailed documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
Engage in team rounds and interdisciplinary meetings to ensure unified and comprehensive patient care
Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Specialty certifications (e.g., neurology, geriatrics) are advantageous but not mandatory
Skills
Prior experience is highly desirable
Proficient in treating patients with diverse medical and mobility challenges
Excellent communication and team collaboration abilities
Familiarity with digital charting and clinical record-keeping systems
