What You Will Do
Assess patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs
Implement physical therapy interventions targeting mobility, strength, balance, and functional improvement
Collaborate seamlessly with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
Guide patients and their families through therapeutic objectives, discharge planning, and home exercise regimens
Accurately document evaluations, treatment plans, and patient progress according to facility and regulatory standards
Engage actively in care team rounds and interdisciplinary conferences to guarantee coordinated patient care
Qualifications
Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited educational institution
Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Specialized certifications (e.g., neurological, geriatric) are advantageous yet not mandatory
Skills
Prior experience is strongly preferred
Proficient in treating patients with complex medical and mobility challenges
Excellent communication and teamwork abilities
Familiar with digital charting and clinical documentation systems
