Description

Conduct thorough evaluations of patients in both inpatient and outpatient settings and design personalized treatment plans

Administer targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and function

Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes

Provide education to patients and families about therapeutic objectives, discharge strategies, and at-home exercises

Maintain comprehensive documentation of evaluations, treatments, and patient progress in accordance with established guidelines

Engage actively in care team discussions and interdisciplinary meetings to promote cohesive care

Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

Current state licensure as a Physical Therapist

CPR certification is preferred

Additional specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory

Skills

Prior experience is highly desirable

Proficiency in treating patients with intricate medical and mobility challenges

Excellent communication and teamwork abilities

Comfortable utilizing digital platforms for charting and clinical documentation
