Description
Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting tailored treatment strategies
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Collaborate effectively with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal outcomes
Guide patients and their families in understanding therapeutic goals, discharge plans, and home exercise routines
Maintain accurate documentation of evaluations, treatments, and patient progress while adhering to regulatory and facility standards
Engage in care team discussions and interdisciplinary conferences to ensure well-coordinated patient care
Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
Valid state licensure as a Physical Therapist
CPR certification preferable
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience in the field is strongly preferred
Able to address patients with complex medical and mobility challenges
Exemplary communication and teamwork capabilities
Proficient in utilizing digital charting and clinical documentation systems
