Description
Conduct evaluations for patients in either an inpatient or outpatient capacity and formulate tailored treatment plans
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Engage with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
Educate patients and their families about therapeutic objectives, discharge processes, and home exercise programs
Maintain accurate documentation of evaluations, treatment plans, and patient progress consistent with facility and regulatory requirements
Actively participate in care team discussions and interdisciplinary meetings to ensure synchronized patient care
Education
Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
State licensure as a Physical Therapist
Preferred CPR certification
Specialty certifications (e.g., neurological, geriatric) are advantageous, though not mandatory
Skills
Prior experience is highly preferred
Able to manage patients with complex medical conditions and mobility challenges
Excellent communication and collaboration skills
Proficient with digital charting and clinical documentation systems
