Description
Conduct thorough evaluations in both inpatient and outpatient settings, crafting personalized treatment plans for each patient
Implement targeted physical therapy interventions focusing on enhancing mobility, strength, balance, and functional ability
Collaborate with a team that includes nurses, physicians, case managers, and other rehabilitation experts to ensure the best patient outcomes
Guide patients and their families through therapeutic objectives, discharge planning, and home exercise programs
Maintain thorough and accurate documentation of evaluations, treatment sessions, and patient progress in line with facility protocols
Engage in care team meetings and interdisciplinary discussions to promote unified patient care
Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
Certifications
Current state licensure as a Physical Therapist
CPR certification preferred
Additional specialized certifications (neurological, geriatric) are advantageous but not mandatory
Skills
A minimum of prior experience is strongly preferred
Expertise in handling patients with intricate medical and mobility challenges
Excellent communication and collaboration capabilities
Proficient in using digital charting software and clinical documentation
