Description
Conduct patient evaluations in either inpatient or outpatient settings, carefully crafting personalized treatment plans
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and functional abilities
Collaborate closely with nursing staff, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
Guide patients and their families through therapeutic goals, discharge plans, and at-home exercise regimens
Document assessments, treatments, and patient progress following established facility and regulatory standards
Engage in interdisciplinary meetings to ensure cohesive patient care
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution
Certifications
Current licensure as a Physical Therapist in Minnesota
CPR certification preferred
Additional specialized certifications (neurological, geriatric) welcome but not mandatory
Skills
Strongly preferred: previous relevant experience
Capacity to manage patients with complex medical conditions and mobility challenges
Excellent communication and teamwork skills
Familiarity with digital documentation and clinical charting systems
