Description
Conduct thorough evaluations of patients in both inpatient and outpatient environments and craft customized treatment plans
Carry out physical therapy interventions concentrating on mobility, strength, balance, and functional capability
Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to improve patient outcomes
Educate patients and their families about therapeutic goals, discharge planning, and exercises to perform at home
Maintain comprehensive documentation of evaluations, treatments, and patient progress following regulatory standards
Engage in care team meetings and interdisciplinary conferences, ensuring seamless coordination of care
Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Additional certifications in areas like neurological or geriatric therapy are beneficial but not mandatory
Skills
Preferred prior experience in the field
