Description
Conduct comprehensive evaluations for patients in both inpatient and outpatient settings, creating customized treatment plans
Implement physical therapy interventions that prioritize mobility, strength, balance, and functional independence
Collaborate closely with nurses, physicians, case managers, and fellow therapists to enhance patient outcomes
Guide patients and families through therapeutic objectives, discharge planning, and recommended home exercises
Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility protocols and regulations
Engage in interdisciplinary rounds and conferences to ensure cohesive and coordinated care
Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
Certifications
State licensure as a Physical Therapist
Preferred CPR certification
Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Preferred previous experience in a similar role
