Description
Conduct thorough evaluations of patients across inpatient and outpatient settings while designing tailored treatment plans
Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall functionality
Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to optimize patient outcomes
Educate patients and their families about therapeutic objectives, discharge plans, and at-home exercise regimens
Maintain proper documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
Engage in care team rounds and interdisciplinary meetings to ensure cohesive patient management
Education
Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution
Certifications
Active state licensure as a Physical Therapist
CPR certification preferred
Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory
Skills
Prior experience is highly valued
A proven ability to care for patients with complex medical and mobility challenges
Strong communication and teamwork capabilities
