Description
Conduct comprehensive evaluations of patients across inpatient and outpatient settings, tailoring individualized treatment strategies
Deliver physical therapy interventions designed to enhance mobility, restore strength, improve balance, and optimize function
Work collaboratively with a multidisciplinary team of healthcare professionals including nurses, physicians, and case managers to achieve the best outcomes
Provide education to patients and their families regarding therapeutic objectives, discharge planning, and effective home exercise regimens
Accurately document patient evaluations, treatments, and progress in line with facility and regulatory guidelines
Engage in care team rounds and interdisciplinary conferences to facilitate coordinated and effective care
Education
Doctorate in Physical Therapy (DPT) or a recognized equivalent degree from an accredited institution
Certifications
Active state licensure as a Physical Therapist
CPR certification is preferred
Specialty certifications (e.g., neurological, geriatric) are a bonus but not mandatory
Skills
Prior experience in a similar role is highly favored
Ability to treat a diverse patient population with complex medical and mobility challenges
Excellent communication and teamwork capabilities
Proficient in the use of digital charting and clinical documentation systems
