Job Responsibilities

Conduct assessments of patients in inpatient or outpatient settings and create tailored treatment plans

Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes

Provide education to patients and families about therapeutic objectives, discharge planning, and home exercise routines

Maintain thorough documentation of evaluations, treatments, and patient progress in compliance with regulatory standards

Engage in care team discussions and interdisciplinary meetings to ensure seamless coordination of care

Qualifications

Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution

Certifications

Current state licensure as a Physical Therapist

CPR certification is preferred

Additional certifications in specialized areas (e.g., neurological, geriatric) are advantageous but not mandatory

Qualifications and Skills

Prior experience preferred
