Description
Conduct assessments for patients in both inpatient and outpatient environments, creating personalized treatment plans tailored to individual needs
Implement therapeutic interventions that enhance mobility, strength, balance, and overall functionality
Engage in collaborative efforts with a multidisciplinary team including nurses, physicians, and other rehabilitation professionals to optimize patient outcomes
Educate patients and their families on rehabilitation goals, discharge protocols, and home exercise routines
Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility policies and regulatory standards
Actively participate in collaboration rounds and interdisciplinary meetings to ensure cohesive care delivery
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
State licensure as a Physical Rehabilitation Specialist
CPR certification is preferred
Additional specialized certifications (e.g., in neurology or geriatrics) are welcomed but not required
Skills
Prior experience preferred, but open to new graduates who show a passion for patient care
Aptitude for treating patients facing complex medical challenges and mobility issues
Exceptional communication abilities and a collaborative spirit
Familiarity and comfort with digital health records and clinical documentation software
