Physical Therapist

Description

Conduct evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment strategies tailored to each individual.

Implement physical therapy interventions that hone in on mobility, strength, balance, and functional capabilities.

Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes.

Provide education to patients and their families regarding therapeutic goals, discharge planning, and at-home exercises.

Maintain clear documentation of evaluations, treatments, and patient progress in line with facility standards and regulatory guidelines.

Engage in interdisciplinary team rounds and care conferences to ensure comprehensive, coordinated care.

Education

Doctorate in Physical Therapy (DPT) or other equivalent degree from an accredited institution.

Certifications

Must hold a current state license as a Physical Therapist.

CPR certification is preferred.

Specialized certifications (such as those in neurological or geriatric therapy) are welcomed but not a requirement.

Skills

Prior experience as a Physical Therapist is strongly preferred.

A proven ability to work with patients facing complex medical and mobility challenges.

Exceptional communication and teamwork abilities.

Competence in utilizing digital charting and clinical documentation platforms.
