Description
Conduct thorough evaluations of patients in both inpatient and outpatient settings throughout the course of their recovery, crafting personalized treatment plans that address individual needs.
Administer tailored physical therapy interventions concentrating on enhancing mobility, increasing strength, improving balance, and optimizing function.
Work collaboratively with nurses, physicians, case managers, and a network of rehabilitation professionals to ensure the best possible outcomes.
Provide education to patients and their families regarding therapeutic goals, strategies for discharge planning, and exercises to be performed at home.
Keep detailed records of evaluations, treatment protocols, and patient progress in line with facility policies and regulatory standards.
Engage in care team meetings and interdisciplinary discussions to support coordinated and comprehensive patient care.
Education
Doctorate in Physical Therapy (DPT) or an equivalent degree obtained from an accredited institution.
Certifications
Current state licensure as a Physical Therapist.
CPR certification is preferred but not mandatory.
Possession of specialized certifications (e.g., in neurological or geriatric care) is a plus, though not required.
Skills
A solid foundation of experience in a clinical setting is strongly preferred.
