Your Role and Responsibilities
Conduct assessments for patients in both inpatient and outpatient environments, designing tailored treatment plans
Implement physical therapy techniques centered on mobility, strength, balance, and functional performance
Collaborate with a diverse team of nurses, physicians, case managers, and rehab specialists to maximize patient outcomes
Guide patients and their families through therapeutic objectives, discharge planning, and home exercise programs
Ensure thorough documentation of evaluations, treatments, and patient progress in adherence to facility and regulatory standards
Engage in care team evaluations and interdisciplinary discussions to promote cohesive care strategies
Qualifications
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Additional certifications (e.g., neurological, geriatric) welcome, but not mandatory
