Description
Conduct comprehensive evaluations for patients in inpatient and outpatient settings, tailoring personalized treatment plans accordingly
Implement effective physical therapy interventions aimed at enhancing mobility, strength, balance, and functional abilities
Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to achieve outstanding outcomes
Provide education to patients and their families regarding therapeutic objectives, discharge plans, and appropriate home exercises
Maintain accurate documentation of evaluations, treatments, and patient progress in alignment with facility and regulatory standards
Engage in interdisciplinary rounds and team meetings to ensure seamless and coordinated patient care
Education
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Specializations such as neurological or geriatric therapy are advantageous but not mandatory
Skills
Prior experience is highly preferred
Proven ability to manage patients with complex medical issues and mobility challenges
Excellent communication and collaborative skills
Proficiency in using digital charting and clinical documentation tools
