Description
Conduct evaluations of patients in both inpatient and outpatient settings and create personalized treatment plans tailored to their specific needs
Implement physical therapy interventions that prioritize mobility, strength, balance, and functional recovery
Work in collaboration with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
Educate patients and their families on therapeutic objectives, discharge planning, and home exercise routines
Maintain accurate documentation of evaluations, treatments, and patient progress adhering to facility guidelines and regulatory standards
Engage in care team discussions and interdisciplinary meetings to ensure a coordinated approach to patient care
Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
Certifications
Current state licensure as a Physical Therapist
CPR certification preferred
Any specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience in the field is strongly preferred
Ability to manage patients with complex medical histories and mobility challenges
Exemplary communication and collaboration skills
Proficiency with digital charting and clinical documentation systems
