Description
Conduct thorough evaluations in either inpatient or outpatient settings and create customized treatment plans for each patient
Implement interventions targeting mobility, strength, balance, and overall function
Collaborate closely with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes
Guide patients and their families through therapeutic goals, discharge plans, and home exercises
Document patient evaluations, treatments, and progress according to established facility protocols
Participate in care team discussions and interprofessional meetings to ensure seamless coordination of care
Education
Doctorate in Physical Therapy (DPT) from an accredited institution or an equivalent qualification
Certifications
Current state licensure as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience in a clinical setting is advantageous
Ability to manage patients with complex medical and mobility-related challenges
Excellent communication and teamwork skills are essential
Proficiency with digital record-keeping and clinical documentation systems
