Description
Assess and evaluate patients in both inpatient and outpatient settings, designing personalized treatment regimens.
Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall functionality.
Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes.
Inform and educate patients and their families about therapeutic goals, discharge planning, and home exercise protocols.
Keep thorough documentation of evaluations, treatments, and patient progress in accordance with facility standards and regulatory guidelines.
Engage in interdisciplinary care team meetings and rounds to ensure cohesion in patient care.
Education
Doctorate in Physical Therapy (DPT) or a related accredited degree.
Certifications
Licensed to practice as a Physical Therapist in the state.
Preferred CPR certification.
Additional certifications in specialized fields (e.g., geriatric, neurological) are advantageous but not mandatory.
Skills
Previous experience in a similar role is highly beneficial.
Ability to work with patients who have complex medical and mobility challenges.
Effective communication and collaborative teamwork skills.
Familiarity with digital clinical documentation and charting systems.
