Assessing patients in both inpatient and outpatient environments, crafting tailored treatment plans that address individual needs.
Implementing therapeutic interventions aimed at enhancing mobility, strength, and overall function.
Collaborating closely with a multidisciplinary team, including nurses, physicians, and case managers, to ensure optimal recovery outcomes.
Guiding patients and families through therapeutic goals, discharge planning, and home exercise strategies.
Maintaining detailed documentation of evaluations, treatments, and patient progress in line with facility standards.
Engaging in team meetings and collaborative care discussions to ensure seamless patient care.
To thrive in this role, you'll need:
A Doctorate in Physical Therapy (DPT) or equivalent from an accredited program.
State licensure as a Physical Therapist, with CPR certification preferred.
Previous experience is highly desirable, especially with complex medical and mobility challenges.
Strong communication and teamwork capabilities, alongside comfort with digital documentation processes.
